If blood pressure is not controlled with losartan alone, a low-dose diuretic can be added. Do not combine losartan with potassium-sparing diuretics due to the risk of hyperkalemia.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Dosage<\/strong>: Dosage depends on the individual patient and should be adjusted based on blood pressure response.<\/span>\n<ul>\n<li><span style=\"font-size: 14pt;\"><strong>Adults with hypertension<\/strong>: Treatment usually starts with 50 mg losartan once daily. The maximum antihypertensive effect is achieved after 3-6 weeks of starting treatment. The dose can be increased up to 100 mg once daily.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Children under 6 years<\/strong>: Not recommended as it has not been sufficiently studied.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Children 6-18 years<\/strong>: The initial dose for patients weighing 20 to 50 kg is 0.7 mg\/kg (up to 25 mg Losartan\/day\/dose). Physicians may increase the dose if blood pressure is not controlled.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Adults with high blood pressure and type 2 diabetes<\/strong>: Treatment usually starts with 50 mg losartan once daily. The dose can be increased up to 100 mg once daily depending on the patient&#8217;s response. Losartan can be used with other antihypertensive drugs (diuretics, calcium channel blockers, alpha or beta blockers) and blood glucose-lowering medications (insulin, sulfonylureas, glitazones, and glucosidase inhibitors).<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Adults with heart failure<\/strong>: Treatment usually starts with 12.5 mg once daily. The dose should be increased weekly (i.e., 12.5 mg\/day for the first week, 25 mg\/day for the second week, 50 mg\/day for the third week, 100 mg\/day for the fourth week, 150 mg\/day for the fifth week) up to the maintenance dose determined by the physician. A maximum dose of 150 mg losartan once daily can be used. In the treatment of heart failure, losartan is often combined with diuretics and\/or digitalis and\/or beta-blockers.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Special Populations<\/strong>: The physician will prescribe a lower dose, especially at the start of treatment, for patients taking high-dose diuretics, those with hepatic impairment, or the elderly (over 75 years).
