- Dosage & Administration:
- Administration: Take once daily. Take the tablet with water, with or without food.
- Dosage:
- Treatment of Hypertension: The usual effective dose is 40 mg once daily. Some patients may only need a 20 mg daily dose. If the target blood pressure is not achieved, the telmisartan dose can be increased to 80 mg once daily. Alternatively, telmisartan can be used in combination with a thiazide diuretic like hydrochlorothiazide.
- Cardiovascular Disease Prevention: The recommended dose is 80 mg once daily. When initiating telmisartan treatment for cardiovascular risk reduction, blood pressure should be closely monitored, and if necessary, the dose of other antihypertensive medications should be adjusted accordingly.
- Renal Impairment: Use is limited in patients with renal impairment or on hemodialysis. A lower initial dose of 20 mg should be used in these patients. No dose adjustment is needed for patients with mild to moderate renal impairment.
- Hepatic Impairment: Contraindicated in patients with severe hepatic impairment. In patients with mild to moderate hepatic impairment, the dose should not exceed 40 mg once daily.
- Elderly: No dose adjustment is needed.
- Children under 18 years: Safety and efficacy have not been established.
- Contraindications:
- Hypersensitivity to Telmisartan or other components of the medication.
- Pregnant women in the first and third trimesters. Breastfeeding women.
- Severe renal impairment. Renal impairment with a serum creatinine concentration ≥ 250 micromol/liter or serum potassium ≥ 5 mmol/liter or CI < 30 ml/min.
- Severe hepatic impairment.
- Biliary obstruction.
- Concomitant use of Telmisartan 40 with aliskiren-containing products in patients with diabetes or renal impairment (glomerular filtration rate (GFR) < 60 ml/min/1.73m²).
- Storage & Quality:
- Storage: Store in a cool, dry place at a temperature below 30°C, away from light.
- Shelf Life: 36 months from the manufacturing date.
